What does it cost?

Do I keep my insurance?
Yes. The membership is separate.
Insurance still covers what it covers.
Hospitalizations, specialists, imaging, surgery, prescription medications, lab work. Membership covers your relationship with Dr. Sharma: complete primary care on Concierge, metabolic and hormone care on the Metabolic & Hormone Membership.
What if I'm admitted?
The hospital team manages inpatient care.

Do I need to leave my current primary-care physician?
That choice is yours. Some members transfer their care entirely; others keep an in-network physician on record. The introductory conversation is the place to talk it through.
What does "direct access" actually mean?
Secure HIPAA-compliant messaging is open and monitored through the day, and we reply same day to non-urgent messages. For anything urgent or after hours, Concierge members reach Dr. Sharma directly through the steps on their member card. In an emergency, call 911.
How often can I be seen?
As often as your care requires. On the Concierge Membership, visits are unlimited with no per-visit fee and no copay, including same or next-day visits for active concerns. The Metabolic & Hormone Membership includes follow-up visits, telehealth and in person, as your treatment requires.
How quickly can I be seen?
Same or next-day for active concerns. Annual physicals are scheduled in advance. Sixty-minute visits, so the calendar holds real capacity.
Are you accepting new patients?
Yes. Dr. Sharma is a board-certified internal medicine physician (a primary-care doctor) and is enrolling new patients now, ahead of the Fall 2026 opening. Founding spots are limited: 10 Concierge members and 10 Metabolic & Hormone members, filling on a first-come basis.
Is this insurance, or a membership?
It is a membership, not insurance. You pay a flat monthly rate directly to the practice for your care here (complete primary care on Concierge; metabolic and hormone care on the Metabolic & Hormone Membership), and you keep your own insurance for hospitalizations, specialists, imaging, surgery, and prescriptions. The membership covers the relationship with your doctor; your insurance covers everything outside it.
Can I do same-day video visits?
Yes. Concierge members reach Dr. Sharma directly by message, phone, or video, and a same or next-day video visit is usually available for active concerns. Some things still need an in-person exam, and he will tell you plainly when that is the case.
Do I have to live in a certain town to join?
No. The practice serves adults across the East Bay along the I-680 corridor, including Walnut Creek, Concord, Pleasant Hill, Lafayette, Alamo, Danville, Orinda, Moraga, Rossmoor, and San Ramon, plus the greater Contra Costa and nearby Alameda County area. The exact office location is shared at enrollment.
What is included in the membership?
That depends on your membership. Metabolic & Hormone ($199/month founding) includes physician-managed GLP-1 weight loss, estrogen, menopause, and perimenopause care, labs coordinated and monitored, dose titration and side-effect management, direct messaging, and follow-up visits. Concierge ($499/month founding) includes all of that plus comprehensive primary care: unlimited hour-long visits with no per-visit fee, chronic disease management, prescription dermatologic care, specialist and hospital coordination, The Annual (a ninety-minute exam, full panel, and written plan), and labs and prescriptions ordered for you.
How much does physician-managed GLP-1 weight loss cost per month?
The Metabolic & Hormone Membership is $199 a month at the founding rate (first 10 members; the rate rises after) and covers ongoing medical management: visits, prescribing when clinically appropriate, labs coordination, dose titration, and direct messaging. The medication itself is billed separately through your pharmacy, insurance, or manufacturer programs. GLP-1 medications (Wegovy, Zepbound) are prescribed to eligible candidates after a clinical evaluation, and not everyone is a candidate.
What does menopause care cost per month?
$199 a month at the founding rate under the Metabolic & Hormone Membership, covering ongoing medical management: visits, labs coordination, dose adjustment, and direct messaging with Dr. Sharma. New patients begin with a $149 initial evaluation, credited toward the first month. treatment is individualized and not everyone is a candidate. Prescriptions are billed separately at your pharmacy.
Are medications included in the membership?
Your membership includes Dr. Sharma’s care within its scope: visits, lab ordering and review, prescribing, and ongoing adjustments. Prescriptions are sent to the pharmacy, and you pay for the medication there. What you pay for it depends on the medication and whether your insurance covers it.
What is not included?
On the Metabolic & Hormone Membership, primary care itself is not included, and medications are billed separately at the pharmacy. Beyond that, a few things sit outside the practice for every member, and we help you coordinate the right care for them: emergency care, urgent care needing imaging or procedures beyond office scope, OB and delivery, pediatric care under 18, and inpatient surgical services.
Are there any add-on fees?
No. The price page is the price page. No upcharges, no add-ons, no surprise referral fees. Any future rate adjustments will be communicated to existing members with sixty days’ written notice. Founding members are locked at their rate permanently.
What is the initial evaluation?
Every new Metabolic & Hormone patient starts with a $149 initial evaluation. This covers a comprehensive visit with Dr. Sharma, appropriate lab work, and a clear recommendation on whether GLP-1 therapy or hormone therapy is appropriate for you. If you move forward with membership, the $149 is credited toward your first month.
What is the founding-member offer?
Founding members lock their rate for the duration of their membership: $499 per month for Concierge (first 10 members; the rate rises after), and $199 per month for the Metabolic & Hormone Membership (first 10 members; the rate rises after). There is no initiation fee, and what you pay up front is credited toward your first month. For Concierge, an introductory conversation reserves your spot; Metabolic & Hormone begins with the $149 initial evaluation.
Which membership is right for me?
If you want Dr. Sharma as your complete personal physician, choose Concierge: it includes everything the practice offers. If you want focused, physician-managed care for weight or hormones, the Metabolic & Hormone Membership covers exactly that, and you can move up to Concierge whenever a spot is open.
When does the practice open?
Fall 2026. Founding members reserve now (Concierge with an introductory conversation, Metabolic & Hormone with the $149 initial evaluation) and begin care together when the doors open.
Do you accept Medicare?
Not at this time. The practice isn’t currently able to take patients who would use Medicare for their primary care.
How do I cancel?
A written notice ends the membership at the end of the current paid month. No cancellation fee. No interrogation.
Can I refer a friend or family member?
Yes. Active members can refer a family member, friend, or colleague for membership at any time. Referred prospects skip directly to the introductory consultation. If a referred patient enrolls, the referring member receives a one-month membership credit applied to the next billing cycle. There is no cap on referral credits.
What is Dr. Sharma's medical training?
Doctor of Medicine from Ross University School of Medicine. Internal Medicine residency at Saint Agnes Medical Center in Fresno, California. Board certified by the American Board of Internal Medicine. Training spanned the full spectrum of internal medicine: outpatient primary care in the clinic, caring for hospitalized patients on the wards and in the ICU, and specialty clinics.
What if my issue is more complex than a typical clinic visit?
As an internal medicine physician, Dr. Sharma is trained for the full range of adult medicine, and his hospital training caring for patients on the wards and in the ICU has prepared him to evaluate complex or serious presentations and to coordinate a higher level of care when it is needed. When hospitalization or a specialist is needed, Dr. Sharma coordinates the referral and stays involved.
What makes this practice distinctive?
The same physician, Dr. Sharma, at every visit. Now enrolling founding members. Direct access to Dr. Sharma, including his direct line on the Concierge Membership. Month-to-month membership with no annual contract. Full-spectrum internal medicine for adults on Concierge; focused metabolic and hormone care on the Metabolic & Hormone Membership. When something needs a higher level of care, Dr. Sharma recognizes it early and coordinates the referral or hospital care; for emergencies, call 911. Can family members join?
Yes. Each adult family member enrolls as their own member at the same monthly rate. Each membership is its own physician relationship: separate chart, separate continuity. The practice serves adults; pediatric primary care is not offered.
Is the membership tax-deductible?
Tax treatment of concierge medicine memberships varies by individual circumstance and changes with IRS rules. Please consult your accountant or tax advisor for guidance specific to your situation. The practice does not provide tax advice.
